Description
At Mercy Medical Center our RN Inpatient Care Coordinators are responsible for coordinating patient care, assessing needs, and developing care plans. They collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care well advocating for the patients' well-being.
What You Will Do
In this critical role you will assist in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.

What You Will Do
In this critical role you will assist in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.
Minimum Qualifications
- Education: Graduate of an accredited college or university affiliated nursing program. BSN graduate preferred.
- Licensure: Current Licensure as a Registered Nurse RN in Connecticut.
- Certification: BLS (AHA/ARC). Certification in Case Management (ACM, CCM) required within one year of employment after meeting minimum requirements.
- Experience: three years of diversified, progressive experience with at least 1 year of recent experience in an acute care facility or public health nurse setting.
- Including an in-depth understanding of and skill in using many nationally recognized medical utilization criteria sets (i.e. MCG, InterQual, and others as needed).
- Requires a solid understanding of managed care methodologies, HCFA/OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities of the Continuum of Care
